New Patient Registration

If you have been previously registered with an NHS GP then all you need is your NHS card (or number- you can get this from your previous surgery if you have lost your card) and proof of address if you have changed your address.

The following can be accepted as proof of address:

  • Utility bill (NOT mobile phone bill)
  • Tenancy agreement

If you have never been registered with an NHS Doctor then you will need to bring in your passport and proof of address as above.

If you are uncertain as to whether you are entitled to NHS care then one of our receptionists will be able to help any queries.

If you are sure you are entitled to an NHS GP then please bring the documention above into the surgery (please try to avoid 8.30-10am as we are extremely busy).

If you would like to save a little time then please click on the pre-registration tab and you can complete the registration form  and medical questionnaire online.

THIS DOES NOT CONFIRM YOU REGISTRATION AND YOU STILL NEED TO VISIT THE SURGERY.

Following registration you must have a new patient check prior to seeing one of the doctors.
